No-nonsense nurse Epiphany Johnson was a force to be reckoned with. A full-figured woman who never hesitated to speak her mind and didn't mince words, Epiphany was an intimidating presence. A dedicated nurse who never tolerated anyone giving less than 110%, she often called out doctors and nurses alike when she felt that they were less than professional. She had a strong moral compass that was never tested more than when her only son, Stanford, decided to go to work for mob boss, Sonny Corinthos.

Epiphany strongly disapproved of her son squandering his education on spying for the mob. Despite her objections to Stan's career choices, Epiphany loved her son fiercely and never stopped trying to convince him to make different choices. Sadly, Epiphany would see her greatest fear realized.

On January 3, 2008, Epiphany was on the phone with her son when Stan was killed in a mob-ordered hit. The hit was ordered by international terrorist, Jerry Jacks, and Jerry had been standing nearby and taken pleasure in watching Epiphany hear her son's murder. The pain of losing her son in such a violent manner had taken its toll on Epiphany.

In February 2008, Epiphany suffered a serious heart attack and collapsed in the hospital locker room. She had just received her son's cremated ashes, and she had been clutching them in her hands when she was found. The doctors were able to save, and Epiphany made a full recovery, but it took her time to let go of her anger towards Sonny Corinthos -- the man her son had worked for. Epiphany was not fond of the hospital gossip and unprofessionalism when her colleague Dr. Patrick Drake had an extramarital affair with another doctor on staff, Dr. Lisa Niles. Patrick's wife Robin, also a doctor at the hospital, and Lisa often feuded, even in front of patients. Epiphany repeatedly reprimanded the trio and reminded them to keep their personal business private.

However, Epiphany showed her softer side after Robin was presumed deceased. Patrick was overcome with grief and he turned to pills. Epiphany refused to let Patrick grieve alone, and she provided him with a combination of tough love and friendship that helped him get his life back on track.

Epiphany had a beautiful voice, and she performed every year in the annual Nurses Ball, where she would eventually meet the man who would steal her heart. She also made friends whom she had lost along the way. Epiphany performed a poignant rendition of You're Not Alone at several notable funerals.

In 2014, Milo Giambetti revealed that he had a crush on Epiphany. Epiphany was flattered, and she agreed to go out with him, even though they were seemingly opposites. Milo was younger, a fitness buff, and he worked for mobster Sonny Corinthos. Despite their differences, Epiphany and Milo were a perfect match, and they quickly fell in love then moved in together.

In September 2015, Epiphany took a second job working for Sonny, helping him recover from a gunshot wound that had left him paralyzed from the waist down. She refused to let him give up, and she took him to the gym as part of his physical therapy. Sonny remained sullen until Epiphany introduced him to "wheelchair boxing." She had Sonny work out on the heavy bag, and he seemed pleased to work up a sweat.

As a new year dawned, Sonny decided to take his recovery to the next level, but Epiphany objected. Sonny locked his wheelchair in place and slowly stood up without any assistance. Shocked, Epiphany's eyes filled with tears of joy. Sonny told Epiphany about his dream of running to help someone in need, and how the dream had made him feel strong and hopeful. That was when Sonny had realized that it was time for him to get back on his feet.

Epiphany's happiness turned to anger when Sonny confided that he hadn't told anyone except Epiphany. She realized that he had hoped to keep his enemies from finding out. Epiphany accused him of being heartless because his family deserved to know the truth. She stormed out, but she eventually forgave Sonny for his deception.

In September 2016, Epiphany suffered a devastating loss when her dear friend Sabrina Santiago fell victim to a serial killer who had been roaming the halls of the hospital. The killer was later revealed to be district attorney Paul Hornsby. He had strangled Sabrina when Sabrina had figured out that Paul had been killing patients and staff members.

A few months later, General Hospital was forced to cut jobs to save money, or risk the doors closing. The nursing staff had been slated to take the biggest hit, so Epiphany had rallied her nurses and staged a "sick-out." However, Dr. Hamilton Finn saved the hospital from closing when he donated all the money, including the royalties, that he had made selling the patent to a lifesaving serum for Blackwood's syndrome.

On June 27, 2019 Epiphany noticed that nurse Bobbie Spencer was distracted. Bobbie confided that she had been diagnosed with Type 2 Diabetes. Epiphany pulled Bobbie into a conference room for a private chat. Epiphany knew the challenges of living with Type 2 Diabetes because she had been diagnosed with it in 2016. Bobbie was stunned because she'd had no idea, but Epiphany admitted that she had always preferred to keep her medical issues private.

Epiphany talked about the challenges of living with the disease, but she and Milo shopped at the local farmer's market every weekend, and she exercised regularly. Epiphany admitted that Milo knew her diet restrictions better than she did, so he helped her stay on track. Bobbie confessed that she was worried because Type 2 Diabetes also contributed to heart disease. Epiphany revealed that she had had a heart attack in 2017, a year after her diagnosis. Luckily, it had been a mild heart attack, but she and Milo had been on vacation at the time.

Epiphany had felt terrible when she had seen the worry in Milo's eyes. That was Epiphany's turning point; she had stopped resenting the illness, and started following the guidelines. Bobbie was touched that Epiphany had shared her story, and she thanked Epiphany for the support.
